2024 Boondocks Basketball Club Sign Up
The player does not need to attend.
PLAYERS NEED TO BE SIGNED UP IN ORDER TO PARTICIPATE AT THE 1ST PRACTICE THIS WEEKEND

What You Need to Do:
1. Pay the Full Club Fee= $275
Cash OR Check Made Out to: Boondocks Basketball Club
There is no payment App at this time.
2. Fill Out and Sign the Club Waiver Form
Print it off from the link above
OR
I will have copies available at the site
3. Sign the AYBT Permission to Treat/Waiver Form
4. Hand in a Proof of Age Document-you can email a pic or scan of the birth certificate to boondocksbastketball@gmail.com
You Need to bring a PHOTOCOPY of a driver's license or birth certificate. I will not be able to make copies at the site.
5. Please bring a list of email addresses of anyone who wants access to the schedule on the GameChanger App.
